Vitamin B12 sublingual and Metformin: interaction, severity and spacing
Vitamin B12 sublingual and Metformin interact because metformin reduces ileal calcium-dependent B12 absorption and alters gut microbiome B12 handling; long-term use lowers B12 status in a substantial minority of patients. Take them with care. Moderate risk.

Mechanism
Metformin reduces ileal calcium-dependent B12 absorption and alters gut microbiome B12 handling; long-term use lowers B12 status in a substantial minority of patients.
What to do
Check B12 at least annually on metformin, and supplement if levels are low-normal.
How to fit both into one day
B12 is mildly activating. Sublingual dosing bypasses the need for intrinsic factor, so timing is flexible but morning is preferred.
Take metformin with food to reduce GI effects. Extended-release forms are usually taken with the evening meal.
Putting Vitamin B12 sublingual in the morning and Metformin in the evening satisfies the 0-hour separation without disrupting either one's absorption window.
